Who We Serve
CareBridge Senior Services supports senior adults, individuals with disabilities, adults recovering from illness or hospitalization, and those who need additional assistance to remain safe and independent at home. We also support family caregivers who need respite, guidance, or help coordinating care for a loved one.
Whether an individual needs assistance with everyday activities, support following a hospital or rehabilitation stay, or help navigating changing long-term care needs, CareBridge works with individuals and families to identify the support that best fits their situation.
Our Approach

CareBridge takes a person-centered approach to care. Support is based on each individual’s needs, preferences, safety, and available resources. Care plans are reviewed and adjusted as needs or circumstances change.
We understand that care needs can look different for every individual and family. Our approach focuses on providing dependable support while encouraging comfort, dignity, and independence. We also work with families to help them better understand available care options and make informed decisions as needs change.
